About the role:
We are looking for an experienced a Support Worker to provide community-based support across the Kempsey region.
In this role, you’ll support people living with disability to engage in their local community, build independence, and achieve their personal goals. This includes assistance with transport, social participation, daily living tasks, and accessing services, while promoting choice and inclusion.
This permanent part-time position offers 48 hours per fortnight
You’ll build genuine relationships with customers and work alongside their families and broader support network to ensure each person’s needs and goals are met.

Your responsibilities:
Support customers to participate in community, social, recreational, educational and employment activities
Assist with developing independence through skill-building, transport, and confidence in community engagement
Provide support with daily living tasks, including meal preparation, medication, budgeting, and personal care as required
Collaborate with customers to develop and progress person-centred support plans based on their goals and strengths
Build strong, professional relationships with customers, families, and key stakeholders
Help customers navigate and access community services and supports
Maintain accurate, up-to-date records using online systems
Identify opportunities to enhance each customer’s community participation and overall experience with New Horizons
About you
Minimum Certificate III / Cert IV in Disability, Community or Mental Health and/or willingness to obtain.
A strong set of values that guides your work, incorporating our organisational values of passion, integrity & respect.
Great computer skills with proven ability to use Microsoft Office and customer relationships management systems.
Pre-employment Checks:
Current Police Check (Issued within the last 3 months)
Current NSW Drivers License
NDIS Worker Check (or willingness to obtain)
NSW WWCC (or willingness to obtain)
